Consumidor:

Antiinflamatórios não-esteróides (Aines) pode causar mucosa dano, o risco de que parece estar relacionadas com a dose e duração da terapia. Sério GI toxicidade, tais como hemorragia, ulceração e perfuração pode se desenvolver em qualquer momento, com ou sem sintomas de aviso, e ocorre em aproximadamente 1% dos pacientes tratados durante 3 a 6 meses e de 2% a 4% dos pacientes tratados por um ano. Estas tendências continuem com maior duração de uso, embora a terapia a curto prazo não é sem risco. Enquanto agentes que seletivamente inibir a ciclooxigenase-2 (por exemplo, inibidores COX-2) são geralmente pensado para ser associado com um risco reduzido de GI toxicidade em comparação com os Aines, eles não foram comprovadas livre de risco. Além disso, há evidências de que os inibidores COX-2 pode retardar a cicatrização de úlceras gástricas, e, provavelmente, para a mesma extensão como tradicionais anti-inflamatórios não esteróides. Assim, a terapia com todos os Aines, incluindo inibidores COX-2, deve ser prescrito com cautela em pacientes com história de úlcera péptica e/ou hemorragia gastrointestinal. Pacientes com uma história que usar Aines têm maior do que 10 vezes maior risco para o desenvolvimento de um sangramento GI em relação aos pacientes com nenhum desses fatores de risco. Cuidado também é aconselhado se Aines são prescritos para pacientes com outros fatores de risco, tais como corticosteróide oral ou anticoagulante usar, uso de álcool, o tabagismo, a idade avançada, e pobre estado de saúde geral. Vigilância especial é necessária ao tratamento de idosos (por exemplo, idade de 60 anos ou mais) e/ou pacientes debilitados, pois são muitas vezes mais suscetíveis a GI toxicidade das drogas e parecem tolerar ulceração e sangramento bem menos do que os mais jovens, mais saudáveis indivíduos. Sempre que possível, especialmente se o uso prolongado, é esperado, o tratamento com não-ulcerogenic agentes devem ser tentados primeiro. Se AINES são utilizados, os pacientes devem ser tratados com a menor dose eficaz durante o menor duração possível, e profilática da terapia com um agente citoprotetor (por exemplo, misoprostol), histamina H2-antagonista do receptor, ou um inibidor da bomba de protões deve ser administrado conforme necessário. Os pacientes devem ser aconselhados a evitar ou minimizar o consumo de álcool durante a terapia com AINES. Três ou mais bebidas alcoólicas por dia durante o uso de AINE pode aumentar o risco de ulceração gastrointestinal e sangramento. Os pacientes devem ser aconselhados a procurar imediatamente um médico se tiver sintomas que poderiam indicar graves do trato GASTROINTESTINAL ulceração ou hemorragia, tais como dor epigástrica, dispepsia, melena, e hematemesis.
