O uso de ciclosporina é contra-indicado em pacientes com artrite reumatóide ou psoríase, com uma função renal anormal. Ciclosporina provoca uma redução reversível no fluxo sanguíneo renal e taxa de filtração glomerular, resultando em aumento da creatinina sérica e de azoto da ureia no sangue. Leve a nefrotoxicidade, geralmente, responde para a redução das doses de ciclosporina. Persistente, crônica e progressiva nefrotoxicidade ocorreu. Renal biópsias dos pacientes podem demonstrar fibrose intersticial, atrofia tubular, global ou segmentar glomerulosclerose, ou liso vascular dano muscular. Maiores doses cumulativas ou elevados de ciclosporina níveis mínimos podem ser associados com o desenvolvimento de fibrose intersticial. Monitorização clínica da função renal e é necessária a diferenciação entre a ciclosporina-a nefrotoxicidade induzida, rejeicao de transplante, e outras causas de perturbações da função renal deve ser determinada antes de ciclosporina ajustes de dose.
