Acyclovir é principalmente eliminado pelo rim. Pacientes com insuficiência renal podem estar em maior risco de neuro - e nefrotoxicidade (incluindo uma maior deterioração da função renal, dano tubular aguda e insuficiência renal) do aciclovir devido à diminuição da droga folga. A terapia com aciclovir deve ser administrado com cautela em pacientes com insuficiência renal. Ajustes de dose são recomendadas em pacientes com moderada a grave disfunção renal.
