O risco de infarto do miocárdio e acidentes vasculares cerebrais, incluindo aqueles associados com o uso de contraceptivos orais e alguns o uso de estrogênio, está aumentada em pacientes com hipertensão. Além disso, estrógenos (e progestágenos) pode elevar a pressão sanguínea e agravam a hipertensão, assim, agravar o risco. Clinicamente significativo da pressão arterial aumenta foram relatados durante a terapia de estrogênio, particularmente em pacientes recebendo altas doses ou tratadas com contraceptivo oral combinações de ter alta progestational atividade. Estes efeitos também aumentam com a duração do tratamento e a idade do paciente. A terapia com estrógenos deve ser administrado com cautela em pacientes com hipertensão preexistente. Os pacientes devem ser monitorados para mudanças no sistema circulatório de estado, e seu anti-hipertensivo regime ajustado ou estrogênio terapia retirado conforme necessário. Em pacientes que necessitam de contracepção, métodos alternativos devem ser considerados por aqueles que são hipertensos, ao longo de 35 anos de idade, e a fumaça.
