Diuréticos tiazídicos pode ser ineficaz quando a taxa de filtração glomerular é de baixo (JOY < 25 mL/min), porque eles não devem ser filtrados para os túbulos renais, o seu sítio de ação. Além disso, diuréticos tiazídicos diminui o JOY e pode precipitar a azotemia renal doença. Efeitos cumulativos podem também desenvolver-se porque a maioria destes medicamentos são excretados inalterados na urina por filtração glomerular e tubular ativa a secreção. A terapia com diuréticos tiazídicos deve ser administrado com cautela em doses reduzidas em pacientes com insuficiência renal. Se a função renal torna-se progressivamente pior, como indicado pelo aumento de PÃO ou creatinina sérica níveis, uma interrupção ou descontinuação da terapia com tiazídicos devem ser considerados.
