Certas condições foram identificadas como causas da redução da teofilina folga. Eles incluem a idade (recém-nascidos e lactentes < de 1 ano, assim como os pacientes idosos > 60 anos) e as seguintes doenças simultâneas: edema agudo de pulmão; insuficiência cardíaca descompensada; cor pulmonale; febre (>= 102 graus por 24 horas ou mais, ou menos elevações de temperatura por longos períodos); gripe; não tratada ou não controlada hipotireoidismo; doença hepática, cirrose ou hepatite aguda; diminuição da função renal em crianças < 3 meses de idade; sepse, com falência de múltiplos órgãos; e choque. A terapia com teofilinas deve ser administrado com cautela em pacientes que se apresentam com um ou mais desses fatores de risco, e a dose deve ser adequadamente reduzida para evitar toxicidade. Mais intensivo de monitorização dos níveis séricos de teofilina concentrações podem ser necessários. A toxicidade é mais provável de ocorrer quando os níveis de exceder a 20 mcg/mL. Casos graves, às vezes, sem aviso prévio, levaram a arritmias cardíacas, convulsões intratáveis, e a morte.
