O trimetoprim é principalmente eliminado pelo rim. A concentração sérica de trimetoprim pode ser aumentada e a meia-vida prolongada em doentes com compromisso da função renal. Dosagem podem ser necessários ajustamentos e modificações devem ser com base no grau de insuficiência renal, bem como a severidade da infecção. Os fabricantes recomendam uma dose de 50 mg a cada 12 horas, em pacientes com depuração de creatinina entre 15 a 30 mL/min e não usar a droga em pacientes com depuração de creatinina inferior a 15 mL/min.
