O uso de inibidores da HMG-CoA redutase é contra-indicado em pacientes com doença hepática activa ou inexplicáveis, elevações persistentes das transaminases séricas. Inibidores da HMG-CoA redutase são extensivamente metabolizada pelo fígado. Diminuição do metabolismo dos fármacos pode levar ao acúmulo e aumento do risco de toxicidade, incluindo anormalidades bioquímicas de função hepática e, raramente, icterícia, hepatite, cirrose, ácidos alteração no fígado, necrose hepática fulminante. A terapia com inibidores da HMG-CoA redutase deve ser administrado com cautela em pacientes com histórico de doença hepática e/ou consumo excessivo de álcool. Uma dosagem inicial mais baixa pode ser apropriada e acompanhamento clínico de fígado de transaminase níveis, de acordo com o produto individual rotulagem pacote é recomendado. Os pacientes que desenvolvem elevados de ALT ou AST níveis durante a terapia deve ser monitorada até que as anormalidades resolver. Se um aumento acima de 3 vezes o limite superior da normalidade persistir, deve-se considerar uma redução da dose ou retirada da terapia.
