O paracetamol é principalmente metabolizado no fígado para inativo formas. No entanto, pequenas quantidades são convertidos pelo menor caminhos para metabólitos que podem causar hepatotoxicidade ou glacial. Pacientes com insuficiência hepática podem estar em maior risco de toxicidade devido ao aumento do menor, a via metabólica da atividade. Da mesma forma, crônica ou uso excessivo de paracetamol pode saturar o principal enzimas hepáticas e levar a um aumento do metabolismo por pequenos caminhos. Lesão hepática grave, incluindo casos de insuficiência hepática aguda, resultando em transplante de fígado e morte, tem sido relatada em pacientes em uso de paracetamol. A terapia com paracetamol deve ser administrado com cautela em pacientes com insuficiência hepática. Monitorização clínica da função hepática é recomendada. Instruir os pacientes a evitar beber álcool enquanto estiver a tomar paracetamol, contendo medicamentos. Os pacientes devem ser avisados para não exceder o número máximo recomendado de dose diária total de acetaminofeno (4 g/dia em adultos e crianças com 12 ou mais anos de idade), e a leitura de todos os medicamentos de prescrição e over-the-counter medicação etiquetas para garantir que eles não estão levando vários paracetamol produtos que contenham, ou verificar com um profissional de saúde se eles não tiverem certeza.

Narcóticos opiáceos analgésicos agentes são extensivamente metabolizada pelo fígado, e vários deles (por exemplo, codeína, hidrocodona, meperidina, metadona, morfina, propoxifeno) possuem metabólitos ativos que ainda são convertidos para inativo substâncias. As concentrações de soro de esses agentes e seus metabólitos podem ser aumentados e a meia-vida prolongada em pacientes com insuficiência hepática. A terapia com opióides devem ser administrados com cautela e iniciadas em doses reduzidas em pacientes com doença hepática. As doses subsequentes devem ser titulada com base na resposta individual ao invés de um fixo de dosagem programação.
