Tricíclicos e tetracyclic antidepressivo (TCAs) ter atividade anticolinérgica, para que os pacientes idosos são particularmente sensíveis. Aminas terciárias, como a amitriptilina e trimipramine tendem a apresentar maior anticolinérgico efeitos de outros agentes na classe. A terapia com TCAs deve ser administrado com cautela em pacientes com doenças pré-existentes que são susceptíveis de ser agravadas pela atividade anticolinérgica, tais como retenção urinária ou obstrução; glaucoma de ângulo fechado, não tratada, a hipertensão intra-ocular, ou descontrolada glaucoma primário de ângulo aberto; e aparelho digestivo obstrutiva transtornos. Em pacientes com glaucoma de ângulo fechado, mesmo a média de doses pode provocar um ataque. Glaucoma devem ser tratados e sob controle antes do início da terapia com TCAs, e a pressão intra-ocular monitorados durante a terapia.
